Select the icon from the blue bar under the form you want to add to your store:



Select the HTML Only tab, select and copy all of the code and paste it into the User Defined Template in Store Design > Page Template Manager



Adding the User Defined Template

When creating the User Defined Template (selected from the Page Type drop-down) you add the Page Name and optional description (Notes). Once you click the Add button, you would then go to Store Design > Page Template Manager > Other and click the template name link, , add the code to that page, and click
